For things like changing positions you might be able to work out some basic touch signals and if you allow people to position your body then you would be ok. But for actually communication I am not sure what you can do. Now what about you communicating with them? Do you have a safeword? It is possible that not knowing what is going on could freak you out so you should be prepared to have a safeword even if your mouth is full. I have heard of ones for when you can't talk that are physical like holding a balloon and letting go is the safeword.

For him asking you questions the finger idea would work, just number the basic questions like are you ok? I like that idea! We used letting go of the balloon once when I was unable to talk. It was kind of cool, because we usually use a step safe word, yellow for back off just a smidge, and red for stop now. Without having an intermediate safe word, it was only stop, or keep going. Don't worry about popping the balloon if you go with that idea. First you could use of those foil type balloons that don't pop easily. Second you could tell you husband to pick up more than balloon just in case. Yeah it might cause a brief stop but you could get going again right away and still have your safe word in place.

Even the Max Light plugs I use With x db attenuation do not muffle much, the mind just resets "normal" and the hearing becomes more sensitive. The ear has about a xdb dynamic range, but that can shift up or down depending on the background noise
